Patient Forms
To save time at your appointment and expedite the check-in process, we request that you complete your forms before your appointment. There are several options for you to choose from.
Option 1 - Forms at the Patient Portal
The most efficient option for completing your forms is via our patient portal. When you access the portal, you will be asked to enter your last name and a security code. If you do not have your security code, please call our office at 478-953-1020, and the telephone receptionist will give it to you.
Optiion 2 - Online Forms
If you have an appointment on the schedule and that data has been sent to Relatient, you may choose to complete your forms online. You will log in with your first and last name and date of birth. If you receive an error, Relatient may not have your appointment schedule data yet. If you just scheduled your appointment, please wait 24 hours and try again.
Option 3 - Download & Complete
Please save the forms below. You can open them and fill them out on your computer. Then please print the forms and fax to our office at 478-953-5406.
